Understanding BMI: What It Means and What It Does Not

5 min read

Body Mass Index (BMI) is a number derived from your height and weight. It is widely used as an initial screening tool because it requires no special equipment and correlates reasonably with body fat levels in most adults. It is not a diagnostic tool and has meaningful limitations you should be aware of.

The Formula

Metric: BMI = Weight (kg) ÷ Height (m)²

Imperial: BMI = 703 × Weight (lbs) ÷ Height (inches)²

Example (imperial): 155 lbs, 5 ft 7 in (67 inches).

BMI = 703 × 155 ÷ (67²) = 108,965 ÷ 4,489 ≈ 24.3

WHO Adult BMI Categories:

• Under 18.5 – Underweight

• 18.5–24.9 – Normal weight

• 25.0–29.9 – Overweight

• 30.0 and above – Obese

What BMI Tells You

For most adults, BMI correlates well with body fat percentage and with health risks like type 2 diabetes, cardiovascular disease, and hypertension. Population-level studies consistently show that BMI categories predict average health risk, which is why clinicians use it as a quick screen.

What BMI Does Not Tell You

BMI has several well-known limitations:

• It cannot distinguish muscle from fat. A highly muscular athlete may have a BMI in the "overweight" range despite very low body fat.

• It does not account for fat distribution. Visceral fat (around the organs) is more harmful than subcutaneous fat. Waist circumference is a better proxy for this risk.

• Age and sex matter. Older adults tend to carry more fat at the same BMI as younger adults. Women typically carry more body fat than men at the same BMI.

• Ethnicity. Some studies suggest different risk thresholds for Asian and other populations.

Other Metrics Worth Knowing

Use BMI alongside:

• Waist circumference: Above 35 inches for women and 40 inches for men is associated with higher metabolic risk.

• Waist-to-hip ratio: Waist ÷ hip. Above 0.85 (women) or 0.90 (men) signals elevated risk.

• Body fat percentage: Measured more accurately by DEXA scan, hydrostatic weighing, or reasonable estimates from skinfold calipers.

• Blood tests: A complete metabolic panel and lipid panel reveal actual metabolic health independent of weight.

Frequently Asked Questions

Should I worry if my BMI is 25.5?

A BMI just above 25 is at the lower end of "overweight" and, by itself, is a mild indicator rather than cause for alarm. Look at the full picture: blood pressure, blood sugar, activity level, diet, and waist circumference. Talk to a doctor if you are concerned.

Is BMI valid for children?

No. Children use BMI-for-age percentile charts that account for normal growth. A child's BMI must be interpreted against age and sex norms, not the adult thresholds above.
